What the video shows

Form 101, the employee card, is the form every employee in Israel signs at the start of employment and again at the start of each year. On it you declare your personal details, your marital and family status, any other income and the tax credit points you are entitled to. It is the basis on which your employer calculates the tax withheld on your payslip, so filling it in correctly is what stops you being over-taxed.

How to do it yourself

  1. Fill in your personal details · full name, ID number, address and contact details, along with the start date of employment.
  2. Declare your marital and family status. Marital status and children affect the credit points you are entitled to.
  3. Declare other income. Mark whether you have income from another employer or another source · this is what determines whether you also need tax coordination.
  4. Claim the tax credit points you are entitled to, attaching supporting documents where they are required.
  5. Sign and date the declaration. Your signature confirms the details your employer relies on when producing the payslip.
  6. The employer keeps the form and renews it yearly · it must be signed at the start of employment and at the start of each year.
